Application process

1. For CCP - Certificate of Catholic Practice - Certificate of Catholic Practice pdf Certificate of Catholic Practice(314 KB)

• Fill out the application in the section "Details of Child (for identification only)".
• Include current contact information (phone number or email address).
• ZInclude a list of schools to which you will be applying (school name, city or postal code).
• Submit the application to the parish office.

2. For other Catholic schools

• Fill out the appropriate form required by the school to which you are applying (please remember: blank forms will not be signed).
• Include current contact information (phone number or email address).
• Include a list of schools to which you will be applying.
• Submit the application to the parish office

Those who only need an electronic version of the application, please contact the parish office by email
